OSFM and OSBI are excited to announce our new and improved Modification Form!
This new form is a one-size-fits-all tool to request fire and building code modifications, as well as accessibility waivers, fire sprinkler waivers for educational buildings, elevator/LULA modifications, and boiler code modifications. You can access the new form at http://www.ct.gov/modform. There is a separate, streamlined form for one- and two-family homes to request modifications to the IRC portion of the CT State Building Code, also available on the same webpage.
State Fire Marshal Lauri Volkert and I have created tutorial videos to walk through the forms, which can be found on YouTube:

The process is mostly the same as before:
1. Download the form from http://www.ct.gov/modform
2. Have the applicant fill in the details of the request and submit separate forms for each code section that needs to be modified
3. Local Fire Marshal or Building Official review and signature (separate forms for each request)
4. Submit it to das.codemodifications@ct.gov
5. We will review and respond.
